If, after reading the membership overview, requirements, and commitments, you wish to apply for membership, you may print the form below and either:

• Fax it to the President of MPVFR at (770) 552-5929, with a cover sheet "recruit application"
• Post it via mail to:

MPVFR Service Application

Note: an application requires the sponsorship of two members of the department, one of whom must be an officer.  To get these, you must have contact with the department at some point either before or during application.  We are open and friendly, come on by.
